Appointments and Cancellations
We request that you provide at least 24 hours notice if you need to cancel or reschedule an appointment. Repeated no-shows or late cancellations may result in the inability to schedule future appointments.
Payment
Payment is expected at the time of service unless other arrangements have been made. We accept Medicare, Medicaid, most commercial insurance plans, and self-pay. Our self-pay rate is $125 per visit.
